Part 1: Safety Waiver

ASSUMPTION OF RISK AND WAIVER OF LIABILITY

Activity: Ave Maker Club - Participation in maker activities including but not limited to 3D printing, electronics, robotics, woodworking, and other hands-on projects.

I understand and acknowledge that participation in Ave Maker Club activities involves inherent risks, including but not limited to:

  • Use of tools, equipment, and machinery
  • Exposure to heat, electricity, and moving parts
  • Physical activity and potential for minor injuries
  • Contact with materials and substances used in maker projects

By signing below, I acknowledge that:

  1. I have been informed of and understand the risks involved in Ave Maker Club activities.
  2. My child has my permission to participate in all Ave Maker Club activities.
  3. I assume all risks associated with participation, including but not limited to falls, contact with other participants, and the effects of weather.
  4. I agree that Ave Maker Club, its organizers, volunteers, and affiliated school staff are not liable for any injury, loss, or damage to person or property.
  5. I agree to release and hold harmless Ave Maker Club and all associated parties from any and all claims, demands, and causes of action arising from my child's participation.
  6. In case of emergency, I authorize Ave Maker Club staff to seek medical treatment for my child if I cannot be reached.

I have read this waiver and fully understand its contents. I voluntarily agree to its terms and conditions.

Part 2: Media Release & Permission Waiver

PHOTO, VIDEO, AND IMAGE RELEASE AUTHORIZATION

Purpose: Ave Maker Club occasionally photographs and videos participants engaged in club activities for promotional and educational purposes.

This authorization grants Ave Maker Club permission to use images, photos, and videos of my child for:

  • Website and social media promotion
  • Marketing materials and brochures
  • School newsletters and announcements
  • Promotional videos and presentations
  • Educational documentation and showcases
  • News media and press releases

By signing below, I acknowledge and agree that:

  1. I grant Ave Maker Club the right to photograph and video record my child during club activities.
  2. I grant Ave Maker Club unlimited rights to use, publish, and distribute these images and videos in any medium (print, digital, web, social media, etc.) for promotional and educational purposes.
  3. These images and videos may be used without compensation to me or my child.
  4. I waive any right to inspect or approve the finished product or any written copy that may be used in connection with the images or videos.
  5. I release Ave Maker Club, its organizers, volunteers, and affiliated school staff from any claims that may arise from the use of these images or videos.
  6. I understand that while Ave Maker Club will make reasonable efforts to use images respectfully, I cannot control how images shared publicly may be further distributed.
  7. This authorization remains in effect unless I revoke it in writing.
